Are Lazarus and Eleazar alts? The names and powers suggest so, but they didn't act like it the first time they met.

There are a few cases of weird quasi-alts between Aurum and Eos because we hadn't invented the idea of real alts yet when we were populating Eos with powered characters. They don't share anything but related names and similar powers.

Is there an attractor in Effulgence for male Sherlocks to be clones and female Sherlocks to be twins, or is that coincidence, or something else?

Fantasia Sherlock is a boy and a magical duplicate, which seems more like a clone than a twin. Mostly it just has to do with tech availability in the setting.

There is a complicated set of conditions and preferences and rules governing what variety of tony-sherlock pair ends up in a particular universe, and "male Sherlocks are clones and female Sherlocks are twins" isn't explicitly one of them, but I wouldn't be surprised if it happened to be arising out of the mess.

For example: I could do a male Tony-Sherlock set who were twins, but I don't like my current male Tony-Sherlock journal pair enough to want to use it more, and I don't especially feel like adding more virtually indistinguishable boy tonies to the pile, so it's unlikely to happen. Also, not being a clone leaves a hole in a Sherlock's characterization which being trans can fill. If I liked the Fantasia Sherlock's journal more, I might make more Sherlock-Tony sets where the Tony was a cis girl and the Sherlock was a trans boy and they were twins. The lack of good icons of young RDJ appears to be a significant contributor to this trend.
